I just completed her Legendary skill and leveled up to maximum. My skill points came back and my last item is gone. This might be a turning point for my gameplay.
Sounds like a server sync issue. What does the quest say currently? You can contact Custom Service and ask to have the item reimbursed. But that takes time to get a reply. I don't think they can do anything if the quest has reset or timed out.
Sorry to hear that. If you do get screwed at least it is one of the easiest legendary quests in the game to redo
